Hospice Medical Social Worker

We are seeking a qualified and compassionate Hospice Social Worker to join our team. The ideal candidate must have experience with Hospice. The Social Worker will be responsible for providing care and support to our Hospice patients and their families, as well as conducting research and developing systems to improve patient outcomes.
*Responsibilities:*
* Assesses the psychosocial status of Hospice patients and families/caregivers related to the patient's medical condition and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
* Conducts assessments of patients' needs and develop care plans
* Provides individual and group counseling to Hospice patients and their families
* Collaborates with healthcare professionals to ensure the best possible care for patients
* Provides information and referral services for patients and families/caregivers regarding practical and environmental needs
* Provides information to Hospice patients and families/caregivers and community agencies
* Serves as a liaison between patients and families/caregivers and community agencies
* Develops systems to improve patient outcomes
* Conducts research to identify best practices in Hospice social work
* Provides education and training to staff on social work topics
* Maintains accurate records of Hospice patient care
* Become the bereavement Manager
* Must have Hospice experience
*Qualifications:*
* Bachelor's or Master's degree in Social Work or related field
* Experience working with adults and elderly populations
* Driver's license and car insurance
* Strong communication skills
* Ability to work independently as well as part of a team
* LCSW or LMSW License
