We are looking for a motivated Licensed Physical Therapist to join our growing home care team. Fellowship Therapy at Home provides services to clients in the comfort of their home or Assisted Living.

*Responsibilities*

● Evaluate clients, develop appropriate treatment goals and methods, implement the physical therapy treatment plan, and complete all related documentation and record keeping regarding these services.

● Instructs client in the skills and techniques required for participation in mobility activities, and evaluates client progress, attitudes, and behavior as related to rehabilitative potential.

● Adapts program according to the needs of the individual client.

● Puts customer service first: ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual’s needs and rights.

● Possesses an understanding of and support of the mission and philosophy of Fellowship Senior Living.

● Provides supervision to Physical Therapist Assistants including case conferencing, co-signing treatment notes, and onsite client supervisory visits.

*Skills*
* Proven working experience as Physical Therapist
* Ability to communicate effectively with clients, physicians, and team members.
* New Jersey Physical Therapist License
* Doctorate, Masters, or BS degree in PT
