*Job Overview*
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including hospitals, clinics, and home health environments. As an LPN, you will work under the supervision of registered nurses and physicians to ensure the highest quality of care for our patients.

*Responsibilities*
* Administer medications and treatments as prescribed by healthcare providers.
* Assist in discharge planning for patients transitioning from hospital to home or other care settings.
* Provide hospice care to terminally ill patients, ensuring comfort and dignity.
* Conduct triage assessments to prioritize patient needs effectively.
* Document medical histories and maintain accurate medical records.
* Support behavioral health initiatives by monitoring patient behavior and reporting changes.
* Perform IV infusions as needed, adhering to safety protocols.
* Engage in family planning discussions and provide education on related topics.
* Offer specialized care for toddlers, ensuring their developmental needs are met.

*Skills*
* Proficient in medication administration and understanding of pharmacology.
* Strong medical documentation skills with attention to detail.
* Experience in discharge planning processes and patient education.
* Knowledge of hospice care principles and practices.
* Ability to perform triage effectively in fast-paced environments such as the ER.
* Familiarity with behavioral health practices and interventions.
* Competence in IV infusion techniques and protocols.
* Understanding of family planning concepts and patient counseling techniques.
* Experience providing toddler care with an emphasis on developmental milestones.
