Overview: As a professional care provider at Grand Brook Memory Care, the Assistant Director of Health Care (ADHC) will be a part of a team that strives to care for and support their residents and their families the same way they would care for their own loved ones.
Job Description: Under the supervision of the community Director of Health Care, the Assistant Director of Health Care will be responsible for the day to day coordination and oversight of all aspects of nursing, restorative and personal care in accordance with current Federal & State Regulations as well as local regulations as assigned by the Director of Health Care. The ADHC is responsible for planning, facilitating and implementing effective, quality resident care and maintenance of applicable records & reports.
Other tasks (not an all-inclusive list) include:
* Develops methods for coordination of nursing services with the assistance to ensure the continuity of the residents’ total regimen of care.
* Monitors and evaluates vital signs, administers medications and performs administrative duties such as medical forms, reports, evaluations, inventory, studies, and charting
* Maintains professional and effective communication while ensuring the residents feel safe.
* Promptly responds to all resident care needs and notifies management immediately of changes in conditions, emergency situations and incidents.
Qualifications: *Education: Completion of a Professional Nursing Program, approved by the state of which you are employed, and licensed to practice as a licensed nurse. * Experience: Knowledge and experience with residents with Alzheimer’s disease and/or related dementia highly desirable.* * Other: Dedication to serve seniors with passion and excellent customer service. Flexibility to perform various duties in service to the residents, the ability to work a flexible schedule including occasional local

travel, evenings, weekends, and “on call” hours. Ability to read, write, speak and interpret English. Must have a valid driver license, good driving record, and current auto insurance. Must meet all health requirements, including TB testing and pass company and state required background checks. Work Conditions & Physical Demands: The physical demands are representative of the essential functions of this job. Ability to exert maximum muscle force necessary to lift, push, pull, move and transfer up to 100 pounds. The noise level in the work environment is usually moderate. Pay: $31.00 - $32.00 per hour
