*Responsibilities:*
* Builds trusting relationship with patients to facilitate access to health care services and meet health needs
* Engages patients to participate in health promotion, maintenance and restoration activities
* Works in partnership with our multidisciplinary team (including providers, nursing, CHWs, care coordinators and others) to advocate for patients and ensure health needs are met
* Assists patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, and other services as needed
* Revises and reprioritizes plan of patient care, as needed; identifies and addresses any barriers effecting a patient’s ability to adhere to plan
* Conducts community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs and educates about health needs
*Requirements:*
* Graduated from an accredited two year nursing school
* Must possess a current NYS Professional Registered Nurse license
* Must have current CPR certification
* The ideal candidate has knowledge of case/care management principles and has the ability to work with a diverse group of patients and understands the impact of social determinants on health

*Hourly rate: * $30 - $40 an hour (based on a full-time, 40 hour work week). Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
*RN Tuition Reimbursement Potential*
Our FQHC status provides a unique opportunity for tuition reimbursement for registered nurses, up to 60% repayment of qualified student loans directly related to a nursing degree. This tuition reimbursement program requires application and is not guaranteed. RNs who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. Additionally, there is potential for an additional 25% repayment for a third year commitment.
*This position offers a sign-on bonus for external hires, paid in installment's throughout the employee's successful first year of employment.*
