We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team in Lafayette, IN. This night shift position requires providing comprehensive nursing care and support to clients in a residential setting. The successful candidate will be responsible for medication administration if needed, maintaining medical inventory, and ensuring high standards of home health care.

*Responsibilities *
* *Patient Assessment*: Conduct thorough assessments of patients’ physical, emotional, and psychological health. This includes monitoring vital signs, evaluating symptoms, and reviewing medical histories.
* *Care Planning*: Coordinate with RN Case management to help develop individualized care plans based on patient needs and medical conditions. This involves setting goals, determining necessary interventions, and coordinating with other healthcare professionals.
* *Medication Management*: Administer medications as prescribed, monitor for side effects, and ensure proper documentation. This also includes educating patients and caregivers about medication regimens.
* *Wound Care*: Provide wound care, including cleaning, dressing, and monitoring for signs of infection or complications.
* *Patient Education*: Educate patients and their families about managing their health conditions, including disease management, dietary guidelines, and home care techniques.
* *Medical Procedures*: Perform various medical procedures such as catheterizations, intravenous (IV) therapy, injections, and other specialized treatments.
* *Monitoring and Evaluation*: Regularly monitor patients’ progress and response to treatments. Adjust care plans as necessary based on these evaluations.
* *Coordination of Care*: Coordinate with other healthcare providers, including nursing assistants, office clinical team and case workers, to ensure comprehensive and cohesive care.
* *Documentation*: Maintain accurate and detailed records of patient care, including assessments, interventions, progress, and outcomes. This is crucial for continuity of care and legal documentation.
* *Emergency Response*: Respond to medical emergencies in the home, providing immediate care and making decisions about whether further medical intervention is needed.
* *Family Support*: Offer emotional support to patients and their families, helping them cope with the challenges of home health care and providing guidance on how to assist in the patient’s care.
* *Safety and Infection Control*: Ensure that all care practices adhere to safety and infection control protocols to prevent the spread of infections and ensure a safe environment for the patient.
* *Compliance*: Ensure that care provided is in compliance with healthcare regulations, insurance requirements, and organizational policies.
* *Advocacy*: Advocate for the patient’s needs and preferences, ensuring they receive appropriate and respectful care.
* *Resource Utilization*: Utilize community resources and support services to enhance patient care and support.

* *Stress Management*: Skills in managing stress and maintaining resilience in a role that can be emotionally and physically demanding.
* *Prioritization*: Skill in prioritizing tasks and managing time effectively to handle multiple patients and responsibilities efficiently.
* *Emergency Response*: Skills in responding to medical emergencies and providing immediate, effective care.

Job Type: Full-time

Pay: $32.00 - $34.00 per hour

Expected hours: 36 – 40 per week
