We are seeking a dedicated *Physical Therapy Assistant* to join our team at our Skilled Nursing Facility (SNF) in Milford, DE. In this role, you will assist the physical therapist in providing high-quality care to our residents, helping them improve their mobility, strength, and overall function.

*Responsibilities:*
* Assist in implementing physical therapy treatment plans under the direction of a licensed physical therapist.
* Work with residents to improve their mobility, strength, and functional skills.
* Educate residents and their families on physical therapy exercises and techniques.
* Monitor patient progress and report to the physical therapist.
* Document patient progress and maintain accurate records.

*Qualifications:*
* Valid Physical Therapy Assistant license in the state of Delaware.
* Strong communication and interpersonal skills.
* Passion for providing quality care in a supportive environment.
