_*Job Summary: *_
A Physical and or Occupational Therapist , and Speech Therapist administers occupational therapy to patients on a intermittent basis in their place of residence. This is performed in accordance with physicians orders and plan of care under the direction and supervision of the clinical Services Manager.

_*Qualifications:*_
* Graduate of a Physical and or Occupational Therapy curriculum accredited of the American Medical Association.
* Currently Licensed and Registered in which state practicing
* Two years preferred. Homecare a plus.

_*Responsibilities:*_
* Improves or minimizes residual physical disabilities of the patient.
* Periodically participates with all other home care personnel in patients care planning.
* Provides prescribed physical or occupational therapy.
* Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
* Consorts with physicians regarding change in treatment.
* Prepares clinical and progress notes.
* Instructs patient/family members in home programs and fine motor movement exercises.
* Helps develop the plan of care and revises as necessary.
* Performs all skilled procedures.
* Writes report to physician regarding the patients progress

Works indoors in Agency office and patient homes and travels to/from patient ho
- Experience in outpatient and inpatient settings
- Proficiency in critical care and acute care environments
- Strong patient care skills with a focus on individualized treatment
- Knowledge of medical terminology, anatomy, and physiology
- Ability to provide compassionate care and support to patients undergoing rehabilitation
