Social Worker

Community Hospital of Anaconda is seeking a full-time Social Worker, regularly scheduled Monday – Friday.

This position is responsible for enhancing, participating and supporting patient care with a focus on quality and safety. Responsible for consultation and direction of Social Services for Pintler Home Options. Develops, organizes and implements programs, policies and procedures, and conducts case work services and counseling. Recommends functions necessary to ensure overall operational viability. Ability to work with staff, physicians, and with patients and families who can be emotionally upset or hostile.

MSW from a college accredited by the Council of Social Work Educators. Three or more years of previous experience in all aspect of Social Service, including Hospice Social Services. Documented training in bereavement counseling required.

Services of these professionals may include, but are not limited to:

1. Assessment of the social and emotional factors related to the beneficiary's illness, need for care, response to treatment and adjustment to care;

2. Assessment of the relationship of the beneficiary's medical and nursing requirements to the beneficiary's home situation, financial resources and availability of community resources.

3. Appropriate action to obtain available community resources to assist in resolving the beneficiary's problem. (NOTE: Medicare does not cover the services of a medical social worker to complete or assist in the completion of an application for Medicaid because Federal regulations require the State to provide assistance in completing the application to anyone who chooses to apply for Medicaid.);

4. Counseling/Bereavement services that are required by the patient; and (must have documented training in bereavement counseling)

5. Volunteer Program development and implementation
