*Responsibilities (for PT):*
* Conduct thorough assessments of patients' physical abilities, medical histories, and home environments to develop personalized treatment plans.
* Provide skilled physical therapy services including therapeutic exercises, manual therapy techniques, gait training, and balance training.
* Educate patients and their caregivers on proper techniques for exercises and activities to promote recovery and maintain functional independence within their home environment.
* Monitor and document patients' progress, modifying treatment plans as necessary to achieve therapeutic goals and ensure patient safety.
* Collaborate closely with physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
* Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
* Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.

*Responsibilities (for PTA):*

* Implement prescribed physical therapy treatment plans, including therapeutic exercises, manual therapy techniques, gait training, and balance exercises.
* Educate patients and their caregivers on exercises and activities prescribed by the supervising Physical Therapist to promote recovery and maintain functional independence within their home environment.
* Assist in monitoring and documenting patients' progress, reporting observations to the supervising Physical Therapist and modifying treatment plans as necessary.
* Collaborate closely with the supervising Physical Therapist, physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
* Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
* Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.

*Requirements (for PT and PTA):*
* Degree in Physical Therapy (for PT) or Associate's degree from an accredited Physical Therapist Assistant program (for PTA).
* Current state licensure or eligibility for licensure as a Physical Therapist or Physical Therapist Assistant in Massachusetts.
* CPR certification.
* Must pass a background check.
* Clinical experience in a healthcare setting, with preference given to those with experience in home care or community-based settings.
* Strong interpersonal skills, with the ability to communicate effectively with patients, caregivers, and healthcare team members.
* Ability to work independently and manage time effectively in a home care environment.
* Compassionate and patient-centered approach to care, demonstrating sensitivity to cultural diversity and the unique needs of the Merrimack Valley and North Shore residents.

*Preferred Qualifications (for both PT and PTA):*
* Experience with electronic medical records (EMR) systems and proficiency in documentation specific to home care settings.
* Knowledge of Medicare and Medicaid regulations related to home health services in Massachusetts.

*Working Conditions:*
* Provides care in patients' homes within the Merrimack Valley and North Shore areas, with travel between appointments required.
* Flexible scheduling allows you to choose your own hours, typically Monday to Friday, to accommodate patients' needs.
* Must have reliable transportation and be able to navigate local communities efficiently.

*Compensation:*
* PT per diem rate:
* Evaluations: $65 per evaluation
* Visits: $55 per visit
