The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Responsibilities:

Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents‘ physical impairments and functional limitations, focusing on mobility, balance, and gait.

Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents‘ functional mobility.

· Section GG Data Collection: Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.

· Treatment: Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents‘ physical function.

· Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.

· Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( InnovaHealth/Net Health/PCC).

· Interdisciplinary Collaboration: Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.

Qualifications:

State licensure as an Physical Therapist.

· EMR and MS office applications.

· CPR/BLS certification.

· Knowledge of Section GG data collection and documentation requirements.

· Strong clinical and interpersonal skills.

· Excellent written and verbal communication skills.

· Ability to supervise assistants, technicians/aids and student/interns.

· Modality Certification +

· Experience in LTPAC/LTC clinical programs are always a plus%{{advertiserId}}% %%{{category}}%%
