    • Reason for Coverage: Physician out on leave
    • Dates of Coverage: As soon as credentialed
    • Select Rate Type Options: Hourly
    • Schedule (Shifts and Hours): 3 days a week 9am-5pm. Ideally Monday, Wed, Thurs. but some level of flexibility on days and times
    • Call Schedule: 8 call nights a month. But this is flexible as well
    • Average Number of Well Check Visits Per Day: about 12
    • Number of Physicians in Practice: 2 and 1 mid level
    • Locums Will Not be Responsible for Supervising Other Health Care Providers
    • Number of Exam Rooms: Each provider has 2 exam rooms
    • Describe Support Staff: Each provider has MA assigned to them daily. 2 front desk and 1 office manger
    • Average Number of Patients Per Day: 15 - 18
    • Average Number of Vaginal Deliveries Per Day: no deliveries needed
