We are currently searching for a Full-Time Social Worker to join our team.

Responsibilities:
  • Serve as a patient, family, and team advocate.
  • Clearly and effectively communicate the value of hospice & palliative care service to other team members, patients and families.
  • Have the desire to work with those at the end of life.
  • Provide professional psychosocial and emotional support.
  • Serve as a resource for the medical team.
  • Participate in the patient care planning process and promote coordination of patient care.

Qualifications:
  • Education: Must have an active Social Work license from an accredited school of Social Work.
  • LMSW or LCSW required
  • At least two years experience in a clinical care setting that may include: hospice, hospital, long-term care, or clinic settings.
  • Great interpersonal skills, both verbal and written.
  • Familiarity with an EMR (Electronic Medical Record).
