About Us: At The Converse Home, we are dedicated to providing exceptional care and support to our residents. We are located in downtown Burlington and we are well known for our warm, welcoming, and home-like atmosphere. Our Memory Care Community is designed to offer a nurturing and safe environment for individuals with memory impairments. We are seeking a compassionate and skilled Clinical Manager to join our team and lead our memory care unit.
Position Overview: As the Clinical Manager of our Memory Care Community, you will play a vital role in ensuring the highest quality of care for our residents. You will manage a small community of 16 residents, supervise the memory care nursing staff, and work closely with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) within the scope of care. This position requires exceptional communication skills, attention to detail, and the ability to maintain a cohesive team environment across all shifts and departments.
Key Responsibilities:
- Oversee the clinical operations of the Memory Care Community, ensuring residents receive the best care possible.
- Manage and supervise the memory care nursing staff, fostering a collaborative and supportive team environment.
- Collaborate with families, healthcare providers, and outside agencies to coordinate comprehensive care plans for residents.
- Maintain accurate and up-to-date documentation, including care plans, medical records, and reports.
- Conduct regular meetings with staff, families, and external partners to ensure effective communication and continuity of care.
- Monitor and evaluate the performance of nursing staff, providing guidance, training, and professional development opportunities.
- Ensure compliance with all relevant regulations, policies, and procedures.
Qualifications:
- Registered Nurse (RN/LPN) with a valid license in the state of Vermont.
- Experience working with the elderly, particularly in a memory care setting.
- Strong leadership and supervisory skills.
- Exceptional communication and interpersonal abilities.
- Attention to detail and a commitment to delivering high-quality care.
- Ability to work effectively under the DON and ADON within the scope of care.
- Proven ability to maintain a cohesive team environment across all shifts and departments.
