Position Summary:The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.

Essential Job Functions:
  • Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery;
  • Assesses the needs, conditions, and desires of assigned program participants;
  • Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc.
  • Assists participants with establishing or re-establishing community support systems;
  • Acts as advocate on behalf of program participants to improve access to vital services;
  • Participates in clinical staffing decisions regarding the participants current strengths and resources, and their need for additional resources;
  • Completes documentation of services provided in a timely manner;
  • Provides input into the patients treatment needs and disposition;
  • Conducts assessments of program participants and provides therapy as needed;
  • Reviews and documents all clinical services and activities;
  • Participates in case conferences involving assigned program participants;
  • Prepares Discharge Summaries, as needed;
  • Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community;
  • Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation;
  • Performs any other duties as assigned.
Certifications/Licensures:
  • CADC or similar certification preferred; Preference will be given certified candidates;
  • Valid drivers license and the ability to obtain and maintain WestCares vehicle insurance is required.
Education:
  • Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred;
  • High School Diploma or equivalent is required;
Experience and Competencies:
  • Experience working with individuals suffering from mental health and substance abuse disorders required;
  • At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred;
  • Ability to define problems, collect data, establish facts, and draw valid conclusions required;
  • Excellent communication skills are required;
  • Ability to exercise good judgment and discretion required;
  • Ability to work well in a team environment required ;
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required;
  • Professional appearance and demeanor required;
  • Must be culturally/linguistically sensitive to populations served;
  • Intake and assessment experience is highly preferred;
  • Bilingual ability is preferred;
  • Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired;
Working Conditions:
  • Work is primarily performed in an office or residential treatment setting;
  • Some outdoor activities and travel between multiple locations will be required;
  • Regular and prompt attendance is required throughout employment.
