Seeking an experienced Registered Nurse to service home infusion in Galveston, TX and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will be responsible for educating patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patient's well-being by providing high quality nursing care.

Responsibilities:
  • Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System.
  • Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
  • Administer ongoing care/infusions for each patient, provide necessary follow-up as directed by the Physician and Clinical Manager.
  • Confer with physicians and pharmacies in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician based on ongoing assessments and as required by policy/regulation.
  • Report patient care/conditions/progress to patient's physician/pharmacy and Clinical Manager on a continuous basis.
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.

Per Diem Infusion Registered Nurse Required:
  • Active In-State RN License with no disciplinary action.
  • A minimum of 1 year Infusion experience. 
  • Current CPR.
  • Valid Driver’s license/ Proof of Insurance.
  • Recent TB results. (with in last year)
  • Immunization Records.
  • Nursing Bag: Blood Pressure Cuff, Stethoscope, Thermometer. 
  • RN must have access to a scanner or fax. 

Preferred Certifications and Experience:
  • Ig-CN, CRNI, OCN.
  • 1 year nursing experience in the ER, ICU, CCU or PACU.

Nurse Responsibilities:
  • Administration and Self Administration teaching of specialty pharmacy infusion therapies to patients with chronic diseases in physician offices, clinics, infusion suites and home settings.

Nurse Experienced preferred:
  • Infusion of IVIG, SCIG and Biologic Response Modifiers.
  • Experience with Peripheral IV starts and CVAD.
  • Experience with both adults and pediatric patients.
