Job Description: Hospice Case Manager

We are seeking an experienced Hospice Case manager to join our team of medical professionals.

Position Overview:
  • The successful candidate will be responsible for providing quality care to patients in hospice settings.
  • The ideal candidate should have be experienced in the Hospice Industry, communication skills, and the ability to work independently and as part of a team.
  • This is an excellent opportunity for an experienced nurse make a difference in the lives of patients and their families.
Key Responsibilities:
  • Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
  • Handles and manages case assignments 
  • Facilitate multiple care aspects (case coordination, information sharing, etc.) 
  • Aid patients in making informed decisions by acting as their advocate 
  • Develop effective working relations and cooperate with the clinical team throughout the case management process
  • Adhere to professional standards as outline by protocols, rules and regulations
  • Completing initial, comprehensive, and ongoing assessments for patients and families to determine hospice needs
