Case Manager (Per Diem)

Dominican Village is a not-for-profit, senior living community and assisted living residence founded in 1992 by the sisters of St. Dominic of Amityville, New York. Its mission is to provide a supportive environment in which the sacredness of life and the dignity of each person are held in the highest regard and to offer its residents a choice of programs and services which foster independence, promote physical, mental, emotional and spiritual health, and maximize opportunities for socialization.

We are seeking a per-diem Case Manager, to schedule and assess residents’ need for additional personal care or healthcare services. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client's health and human services needed; additionally, the incumbent must maintain strict adherence to all DOH regulations including privacy laws and will work as part of a team.

Candidates must have Bachelor’s degree from an accredited college/university with major work in nursing, social work or human services, and a minimum of 2-4 years’ experience serving the geriatric population. First Aid/AED certified or able to become certified also necessary. LPN a plus.

Job Description:
  • All work and employee interaction among themselves and the residents will be carried out in a manner that is reflective of the values of DOMINICAN VILLAGE. This includes respect for the dignity of each person and a commitment to a continuance of independence of the residents, fostering quality of life with growth and interaction, respect for the rights of residents to privacy and choice, and ensuring a secure and welcoming environment.
  • Attend regularly scheduled meetings with Director of Social Services to discuss cases and clarify and evaluate objectives in supporting the residents in maintaining their independence of function and personal choice.
  • Assists in performing resident assessments and prepares service plans; monitors capability of the programs to meet the residents' needs; provides appropriate information for discussion by Resident Status committee. Timely completion of required paperwork, ie, 4397.
  • Orients and monitors new residents in the daily routine. 
  • Updates the EMERGENCY records when necessary and conducts a semi- annual audit of all EMERGENCY files to verify accuracy of the information.
  • Responds to emergencies and initiates appropriate follow-up with careful attention to the accurate completion of the unusual occurrence reports and Case Management progress notes.
  • Communicates with Hospital/Rehab and/or Social Services to facilitate resident  discharge process with appropriate home care services; observes private duty assistance in accordance with Dominican Village policy. 
  • Responsible to identify and report any situation at Dominican Village that may result in injury or loss for any resident, employee, volunteer and/or visitor. This responsibility includes the identification and reporting of any risks or incidents that may affect the business practices and successful operation of Dominican Village. 
Education:
  • Bachelor’s degree in social sciences, social work, nursing, or a related field.
  • 2+ years of work experience in public health or related field.
  • 1+ years of current case management experience.
  • Strong organizational, time-management, and follow-up skills.
  • Must possess excellent written and verbal communication skills with a high sense of professionalism.
  • Proficient in Microsoft Office Suite (Excel, PowerPoint, Word, and Outlook), and CRM software preferred.
  • One year experience in home care or assisted living industry.
