Job Overview:
As a Case Manager at Stonegate Center, you will play a crucial role in the care and support of individuals battling addiction. You will work closely with clients, their families, and a multidisciplinary team to develop and implement personalized treatment plans aimed at achieving sustainable recovery outcomes.

Responsibilities:
  • Conduct comprehensive assessments of clients' physical, emotional, and social needs.
  • Develop individualized treatment plans in collaboration with clients and their support network.
  • Coordinate and monitor the delivery of services and interventions to ensure continuity of care.
  • Provide counseling, support, and education to clients and their families.
  • Facilitate group therapy sessions focusing on addiction recovery and relapse prevention.
  • Maintain accurate and up-to-date documentation of client progress and treatment plans.
  • Collaborate with healthcare professionals, community resources, and external agencies to optimize client care.
  • Participate in case conferences, team meetings, and continuing education activities.
  • Adhere to ethical and professional standards in all aspects of job performance.

Qualifications:
  • Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field.
  • Certified Addiction Counselor (CAC) or equivalent certification preferred.
  • Experience working in addiction treatment or related healthcare settings.
  • Strong understanding of addiction recovery principles and evidence-based practices.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively in a multidisciplinary team environment.
  • Compassionate, empathetic, and nonjudgmental approach to client care.
  • Knowledge of relevant laws, regulations, and ethical guidelines in addiction treatment.
