The ACT Team Lead establishes and maintains oversight and direction for the clinical and operational leadership of the multi-functional team to which they are assigned. They are responsible for maintaining and documenting regulatory compliance efforts of the team., providing supervisory guidance and support to all employees of the team, including but not limited to coaching, counseling, corrective actions, and hiring and firing of staff. They are also responsible for the fiscal security of the program which they lead, including maintaining proper census levels to ensure the fiscal viability of the program, monitoring, and controlling expenses of the team, and ensuring the proper use and control of program funds. This is a $70,000 salaried position with competitive on-call stipends and a $5,000 sign-on bonus ($2,500 at 90 days, $2,500 at 6 months).

ACT Team Lead:
  • Develops initial assessment and treatment plans for all clients and consumers admitted to the program and assign team members to them.
  • Evaluates consumers on an ongoing basis for planning and coordinating treatment activities to respond to changing needs.

Directly supervises assigned personnel as follows:
  • Assists in the selection of new personnel as needed. 
  • Trains and supports the staff in the completion and review of assessments and the development of effective care and/or support plans as appropriate.
  • Reviews employee performance on a regularly scheduled basis. 
  • Organizes, schedules, and distributes work among assigned personnel. 
  • Keeps personnel informed of pertinent policies and procedures affecting the program and/or their jobs. - Administers policies and procedures as established by RHD. 
  • Assures and guides on-call crisis intervention is available to program participants 24-7. 
  • Assists and guides staff in the following essential activities: linking and gaining access to outside services. - Monitoring and assessment of service delivery. 
  • Problem resolution.
  • Informal support network building. 
  • Rehabilitation skills training. 
  • Peer Support Services
  • Supervise caseload with individual team members as well as with the team on a regular basis and maintain a caseload of their own.
  • Supervises medical record maintenance to ensure compliance with regulatory bodies.
  • Develops and maintains an efficient scheduling system for program personnel, ensuring the needs of the program are met.
  • Works with the assigned financial manager to maintain the fiscal viability of the program through cost control and the development of revenue.
  • Other duties as assigned.

Required Experience:
  • A minimum of three (3) years experience directly serving adults in Mental Health, Intellectual Developmental Disabilities, Substance Abuse, Juvenile Justice, Health Care, Vocational Rehabilitation, or similar programs, including two (2) years of community-based treatment and one year of supervisory experience. 

Required Education:
  • A Graduate Degree in Psychology, Psychiatry, Social Work, Nursing, Rehabilitation, Education, or any Graduate Degree in the field of human services.

Lehigh Valley ACT's recovery-focused, multidisciplinary, assertive community treatment programs provide intensive, individual support for people in Lehigh and Northampton counties who are working to overcome the barriers to recovery from mental illness. Assertive Community Treatment Teams provide Psychiatric assessment, medication education, and monitoring, treatment for co-occurring disorders, Dialectical Behavioral Therapy, psychosocial services and education (including Wellness Management and Recovery, SAMSHA evidence-based IMR), and intervention to help strengthen the individual’s networks. Blended Case Management Services affords the collaborative coordination of services and resources supportive of recovery and growth. Individuals work with case managers to increase personal and financial independence, overcome barriers compromising access to treatment, build and expand capabilities in roles and relationships within the community, and, acquire skills to support self-determination and self-management of the symptoms or challenges related to their illness. HUD funding allows LVACT to respond to the residential needs of participants who have been diagnosed with serious, persistent mental illnesses and have histories of homelessness.
