To provide:
  • Evaluation, Treatment, Re-Evaluation, Discharge Planning.
  • Collaboration with Interdisciplinary team.
  • Complete Documentation.
  • Supervision of PTA

Requirements:
  • Authorized to work in NJ, USA without need for sponsorship.
  • CPR/BLS certificate current.
  • Willing to work in the Home health setting.
