About Support Broker Services LLC:

Support Broker Services, LLC is an IRIS vendor assisting participants in planning, securing and directing the supports they need to live an independent life. Participants may use their IRIS funds to purchase the supports of a Broker. Support Broker services have been an approved IRIS service throughout Wisconsin counties. SBS is proud to be supporting IRIS and private pay participants throughout Wisconsin Services we offer: Helping participants deal with Social Security (Auth rep services); Assistance with personal finances; Assistance with Medicaid & FOODSHARE benefits; Liaison with healthcare providers; End of life and funeral planning; Coordinating complex issues; Transition from high school to adult services; Community integration; Finding appropriate housing and Recruiting direct care workers. Social Outings: The Cool Crew (TCC) will provide structured recreational activities (bowling, movies, zoo, bingo, etc.) in the Madison area during the mid late afternoon. The group will meet once per month. The Breakfast Club meets once per month at various locations around Madison. The Lunch Club offers an ongoing social opportunity for individuals with disabilities to connect and build long lasting friendships. The group meets every other month at various restaurants around the Madison Area. Other services that are identified by the individual & IRIS consultant.
