Hospice Care Liaison

We are currently searching for 2 Care Liaisons to join our team in the Collin County and Central Dallas County areas.
The primary functions of this role include:
- Must be able to Identify referral sources to promote census growth
- Must be able to identify, acquire, and nurture relationships within the service area so they offer HOPE as a hospice choice
- Must be able to provide education on hospice care to referral sources and the general community
- Must use data to identify and track referral trends in order to develop referral source avenues
- Must have good time management to be able to attend meetings to discuss wins, improvement areas, and growth opportunities
- Demonstrate a comprehensive understanding and ensure compliance with local, state, and federal laws relating to the promotion of the agency
- Must have strong and effective communication skills
- Must have the ability to build mutually beneficial personal relationships that contribute to team goals and organizational mission
- Must have strong analytical and organizational skills
- Must have a valid Drivers License in good standing
- Must have reliable transportation
- Must be able to provide 2 years of successful and scalable sales experience in hospice. Other healthcare field sales experience will be considered.
- Bachelor's Degree preferred but not required
