Physical Therapist (Home Health) DIRECT HIRELocation: Portland, OR (Hybrid)Salary: $110k SummaryThis role involves providing compassionate, in-home physical therapy to help patients restore mobility, relieve pain, and maximize their physical function. As the patient’s partner in recovery, the PT follows a physician's plan of care, leading therapeutic exercises, muscle reeducation, gait training, and other functional training activities to help patients reach their personal best. This role emphasizes accurate record-keeping and collaboration with the home care team to ensure continuity of care and adherence to high standards. Flexibility, mileage reimbursement, and a supportive work environment are offered, making this a rewarding opportunity for licensed PTs with a dedication to improving patient lives.  Responsibilities
  • Physical Therapy Services: Deliver in-home physical therapy according to a physician’s written plan of care
  • Patient Treatment: Administer treatments to relieve pain, improve function, and optimize physical performance
  • Therapeutic Exercises: Direct and assist patients in therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training
  • Record Keeping: Maintain clinical records and conference notes, ensuring timely incorporation into patient records
  • Team Collaboration: Attend Home Care Team meetings and participate in in-service education programs to support professional development and patient care quality
 Requirements
  • Licensure: Licensed Physical Therapist within the state
  • Educational Qualifications: Doctorate or Master's (MS) degree in Physical Therapy
  • Experience: Minimum of one (1) year of experience as a Physical Therapist, ideally in home health settings
  • Certifications: Current CPR certification
  • Driver’s License and Vehicle: Valid driver’s license, auto insurance, and a reliable vehicle for home visits
