Title: Registered Nurse - Behavioral Health Program Nurse
Location: Primarily Brazoria County at the Angleton and Alvin clinics; Travel to Galveston County as requested at the Texas City and Galveston Island locations

Summary:
Reporting to the Integrated Healthcare Services Nurse Manager & Onsite Program Manager, the Behavioral Health Registered Nurse performs direct patient care & healthcare administrative work within outpatient clinic setting. The location is primarily Brazoria County at the Angleton and Alvin clinics (schedule to be determined) with travel to Galveston County as requested. Duties include coordination of medication clinics and follow-up of adult population diagnosed with severe & persistent mental illness receiving complex pharmacological management and psychiatric diagnostic evaluation services. Provides patient education & guidance about medication and possible side effects. Additional duties include provision of other medication-related services & psychosocial rehabilitative supports to facilitate recovery as outlined in the Texas Resiliency & Recovery service array. As a part of care coordination, provides follow-up to prescriber orders, including UDS and other lab testing and referrals to internal/external provider(s) as clinically appropriate. As an RN and trained Qualified Mental Health Professional, can assist with the provision of crisis intervention services when clinically warranted, including safety monitoring of patient for the occasion continuous visual contact is necessary. In partnership with treatment team members, is responsible for ensuring quarterly & semi-annual achievement of identified HHSC performance contract measures with goal of improving patient health outcomes. Adheres to GCC training requirements as well as state and federal regulations, including program billing/clinical documentation practices and other contractual obligations and requirements.

Essential Job Duties:
- Supervisory: Where applicable and as agreed upon, clinical supervision of unlicensed nurse assistants, LVN's, and/or other staff to whom medical duties are delegated.
- Computer Skills: Requires skills necessary to use electronic medical record, Microsoft Office programs and e-mail. Proficiency in computer skills, including maneuvering in the SmartCare Electronic Health Record.
- Equipment: Equipment used in performing work required of this position includes cell phone, desk/laptop computer & printer, key to facility/office & controlled access doors. Ability to use equipment necessary for the provision of telemedicine services.
- Reasoning: Duties require the ability to interpret complex policies, guidelines, and TAC, analyze processes and weighing factors to make critical decisions. Duties require the ability to determine the best resolutions to various issues.
- In Service Training: Completes all initial trainings (New Employee Orientation, Nursing Orientation/Competency Checklist, PMAB, First Aid, CPR) within 60 days of hire; obtains required updates/recertification as required, participates in other continuing education trainings as required. In-person initial training is predominantly on site in Texas City.
- Productivity Review: Each workday, responsible for monitoring SmartCare dashboard for in-progress events & co-signatures needed to ensure timely encounter & billing claim submission.
- Must maintain a safe driving record to ensure continued coverage under the Center's insurance carrier.
- Must have reliable transportation, maintain current/valid Texas driver's license; must have driving record insurable by the Center's insurance administrator; must adhere to the GCC Driver Policy.
- Demonstrates thorough knowledge of behavioral healthcare service delivery and works jointly with program staff, including contracted & co-located physical healthcare provider (Community Health Network) to identify and serve individuals with co-occurring physical health needs. Assists all patients through the healthcare system by acting as a patient advocate and navigator utilizes identified evidenced-based practices to conduct comprehensive, preventative screenings for patients (phq9, C-SSRS, biometric screening, OUD, UDS, and other screening as identified) & records results in patient's EHR Collaborates with contracted physical healthcare team to include embedded nurse practitioner, medical assistant, care coordinator RN, and assigned support staff. When called upon, responsible for monitoring and assisting medication support staff with the tracking of manufactured medications as well as shelf samples
- Assist in the delivery of comprehensive behavioral health services to Center-established patients (Essential). Demonstrates proficiency in all aspects of nursing care.
- Facilitates patient medication management & education based on TRR evidence-based protocols to include education & guidance about medications & possible side effects through use of HHSC approved materials (MHSA) as well as medication related services as a part of psychosocial rehab services to appropriately authorized individuals as outlined in TAC Title 25 Chapter 416 Rule 416Provides patient education &ensures appropriate consents are obtained as outlined in TAC Title 25 Chapter 414 Rule 414.405; assists with completion of medication consents annually or as indicated utilizing GCC/medical director-approved patient informed consent in support of prescribing provider, conducts medication reconciliation by reviewing & comparing patient's complete medication regimen at time of follow-up visit post inpatient admission with current outpatient medication regimen to avoid inconsistency across transition in care.
- Where qualified & within scope of practice, provides crisis intervention services & assists patients, families, and staff in crisis resolution responsible for the accurate and safe administration of injectable medication. Participates in the coordination of the medication clinic.
- Monitors patient progress in relation to the Person-Centered Recovery Plan (Essential).Maintains contact with patient and significant others.
- Communicates patient's progress and/or changing needs with the patient responsible for providing input in the Person-Centered Recovery Plan (PCRP) as well as co-signing plan as member of the treatment team
- Documents progress according to the Center's clinical documentation guidelines & EHR procedures responsible for monitoring/scheduling of lab studies and injection administration
- Utilizes clinically sound judgment in developing relationships with patients & service providers (Essential)Decision making reflects knowledge of current trends in mental health services coordinates patient care by serving as liaison between patient, prescriber, and other identified health care workers. Follows & utilizes as a resource guide, the GCC Center Medication Policies & Procedures and the Red Medical Resource Manual Stays current on treatment modalities through continuing education and staff development
- Documents services provided according to current records, policies & procedures (Essential)Same-day documentation is highly encouraged; documentation/billing must be entered accurately and no later than 2 business days following service delivery. Participates in quality assurance initiatives internal within agency as assigned. Establishes baseline data and assists with tracking of vital health indicators gathered at regularly scheduled psychiatric outpatient visits as part of BHCQMs and other outcome measures

- Regularly participates in quarterly Behavioral Health Provider & Nursing Services meetings. Attendance at quarterly provider meetings as evidenced by sign-in sheet
- Abides by all Center policies & procedures regarding training objectives and requirements by maintaining current all staff development training. As evidenced by 100% compliance with training requirements. Maintains current licensure with Texas Board of Nurse Examiner. Maintains all levels of training pursuant to GCC and HHSC training guidelines. Pursuant to the Nurse Practice Act, supervision of Vocational Nurse states that the practice of vocational nursing must be performed under the supervision of a registered nurse, physician, physician assistant, podiatrist, or dentist supervision of Vocational Nurse. All LVNs will receive periodic supervision as outlined in the Center's Delegation of Certain Medical and Nursing Acts policies & procedures. RNs may be asked to provide supervision/oversight of identified LVN(s)All licensed nurses are required to review &familiarize themselves with the Center's Medication Policies & Procedures, to include the Delegation of Certain Medical and Nursing Acts & the Red Medical Resource Manual
- Maintains confidentiality regarding persons served, records, personnel, and other sensitive & confidential information to comply with Center, state, and federal policy. Follows Center policies regarding the confidentiality & disclosures regarding persons served records, personnel files & any other sensitive & confidential information. No confirmed cases of privacy or confidentiality violations

Required Education:
- Requires graduation from an accredited, Texas Board of Nursing approved RN education program.
- Knowledge and experience working in behavioral health care setting.
- Prefer experience working with dually diagnosed mental health and substance use diagnosis and Spanish speaking is desirable.

- Receives administrative & clinical supervision from the Integrated Healthcare Services Nurse Manager with onsite supervision provided by identified Program Manager.
