Position Summary:The Functional Medicine Health Coach/ Patient Success Coordinator is a dual-role position designed to ensure an exceptional patient experience while providing personalized coaching to support patients' health and wellness goals. This role focuses on patient engagement, education, and support, helping patients navigate their healthcare journey and achieve optimal outcomes through the principles of functional medicine.

Key Responsibilities:

Patient Onboarding:
  • Facilitate the onboarding process for new patients, ensuring all necessary documentation and information relevant to patient care. Patients will be provided with a specific health coach questionnaire. 
  • Once patients are established and identified as needing further accountability and support, provide detailed explanations of the functional medicine approach and answer any initial questions they may have with an introductory call. 

Patient Education and Support:
  • Conduct educational sessions and provide resources to help patients understand their treatment plans.
  • Offer ongoing support and encouragement, helping patients adhere to their personalized wellness plans.
  • Develop and deliver personalized coaching programs based on functional medicine principles to address individual health concerns and goals.

Health Coaching:
  • Work one-on-one with patients to set achievable health goals and develop actionable plans.
  • Use motivational interviewing and behavior change techniques to support patients in making sustainable lifestyle changes.
  • Monitor patient progress and adjust coaching strategies as needed to ensure success.

Coordination of Care:
  • Help coordinate care between different healthcare providers.
  • Ensure seamless communication between patients and their healthcare team.

Patient Advocacy:
  • Act as a liaison between patients and medical staff to resolve any issues or concerns.
  • Advocate for patients' needs and preferences in their treatment plans.

Data Management:
  • Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system.
  • Track patient progress and outcomes, preparing reports for the healthcare team.
  • Provide relevant data on patient outcomes to management.

Continuous Improvement:
  • Gather patient feedback and participate in quality improvement initiatives.
  • Stay informed about the latest developments in functional medicine and patient care best practices.

Qualifications:
  • A bachelor’s degree in health sciences, nursing, psychology, nutrition, or a related field is preferred.
  • NBHWC Certification - Functional Medicine Coach is preferred but not required
  • Minimum of 2 years of experience in healthcare: health & wellness coaching 

Skills and Competencies:
  • Strong interpersonal and communication skills, with the ability to build rapport with patients and healthcare professionals.
  • Excellent organizational skills and attention to detail.
  • Proficient in using EMR systems and other healthcare technology.
  • Empathy, patience, and a passion for helping others achieve their health goals.
  • Knowledge of functional medicine principles and practices.
