SUMMARY STATEMENT:
The Case Manager, under the supervision of the President, Vice President of Programs and GRYD Supervisor, will conduct participant outreach and recruitment, linkage to extended services, case plan development, implement diversion activities, and client advocacy services to participants including but not limited to: individual sessions, family meetings, team meetings, and court advocacy.
DUTIES AND RESPONSIBILITIES:
CONDUCT OUTREACH, ADVOCACY, AND A REFERRAL NETWORK FOR PROJECT SERVICES:
- Work with participants that include but are not limited to gang affiliation/involvement.
- Contact service providers and school personnel in contracted areas and provide information regarding project services.
- Provide referrals for youth and families to extended services.
- Communicate effectively with clients from diverse language and cultural backgrounds.
- Evaluate calls and walk-ins from potential clients and determine appropriateness for GRYD.
- Conduct all screening, intake assessments, case management
- Provide support and advocacy services for program participants, including attending probation hearings, school meetings, court, and DCFS.
PLAN, DEVELOP AND IMPLEMENT PROGRAM ACTIVITIES:
- Plan and provide field trips, social and recreational activities, and educational outings for program participants.
- Develop and maintain strategies for client referrals.
- Support community functions (i.e., health fairs, forums, etc.).
- Provide transportation on an as-needed basis to and from events and meetings for program participants.
- Facilitate small and large group sessions.
- Create, develop, and implement a group curriculum to address emerging needs.
- Implement the plan, individual, group, and family sessions.
CASE MANAGEMENT:
- Maintain participant files orderly, document activities, and other contract-mandated documents.
- Coordinate support services.
- Attend CIS and GRYD meetings as assigned.
- Manage and maintain a 20-client caseload.
QUALIFICATIONS & EXPERIENCE:
REQUIRED:
- Bachelor's degree in a related field or equivalent experience in the area of case management.
- Permanent resident or U.S. citizen; proof of work eligibility upon hiring.
- Valid Class C Driver's license, California. Minimum automobile insurance and an appropriate motor vehicle.
- The ability to work with people of diverse cultural, educational, social, and economic backgrounds.
- You have demonstrated knowledge and skill in computer operations.
- Complete a physical examination and TB test within one (1) week of the date of hire.
- Pass a criminal background check to work with children and adolescents.
- Bilingual in Spanish is preferred.
WILL BE CONSIDERED:
- At least three years of recovery of substance use as of the date of the application.
