Case Manager Assistant

We are seeking human service workers who are interested in becoming a Community Inclusion Specialist (CIS) to support adults with developmental disabilities and/or autism in the community. The CIS creates and facilitates outings designed to promote independence and confidence. Other objectives are to increase community involvement, develop new skills, build social ties, and connect people with relevant opportunities to help meet their larger goals. Participants may spend time in the community engaging in anything from cultural activities, social/recreational activities, life skills-based excursions, job exploration, volunteer opportunities, and more. The CIS may accompany up to 5 people at a time in the community, using public transportation or in their own personal vehicle.
We have both full-time and part-time positions available, and the typical hours are 10 to 6.
Essential Duties and Functions:
- Supervise and oversee participants in the community on a daily basis to achieve their personal goals and take part in activities and/or events.
- Meet the basic needs of our participants while in the community. This may include assistance with other Activities of Daily Living (ADLs) as required by the participant.
- Implement ISPs (Individual Support Plans) and behavior plans, as needed.
- Maintain sufficient knowledge of participants’ ISPs and personal needs required to work with them in a meaningful and productive way.
- Maintain tracking of receipts and expenditures for reconciliation.
- Complete all required paperwork, including but not limited to case notes on a regular and timely basis.
- Drive personal vehicles or use public transportation to assist individuals in accessing the community.
- Attend to basic first aid, when needed.
Required Experience:
- One year of experience with people with developmental disabilities.
Required Education:
- High School Diploma/GED, Bachelor’s degree preferred.
Required Qualifications:
- Basic Computer skills using Microsoft Products. Valid driver’s license, access to a vehicle, and clean driving record.

Salary / Compensation:
- Competitive & Generous pay at $22/ hour. A $1,000 signing bonus for full-time, and a $500 signing bonus for part-time. The bonuses are paid in two increments- after 3 months and after 6 months of employment.
