Hours: Monday-Friday, 8:00am-4:30pm (40 hours)
General Duties:  The primary function of the Day Program QIDP is to create activities for persons with disabilities who attend day program that enhance their vocational skillset in preparation and enhancement of their community integrated employment. Day Program QIDP is responsible for the caseload of all persons attending day program including, but not limited to: Attendance at IDTs and annual meetings, upkeep of client files, communication with all stakeholders. 

Essential Functions:     
  • Develop and implement clients’ individualized employment plans.
  • Maintain a consistent flow of communication between Community Employment Services and other Clearbrook programs as well as with client guardian. 
  • Plan community-based outings for Day Program clients with client input including, but not limited to: Volunteer opportunities, shopping trips, tours of community job sites .
  • Plan the day-to-day activities for the 1815 day program. Communicate schedule with the CES department, clients, and guardians.
  • Supervise the daily activities of the day program.
  • Establish natural supports, communication skills, enhanced social skills and the completion of tasks in the employment setting.
  • Adhere to all policies and procedures established for Clearbook, the Community Employment Services Programs and those of all governing bodies. 
  • Complete mandatory training annually.   
  • Acts as QIDP for community clients, coordinating/attendance IDTs, and intake/discharge staffings. Writing and implementing individual programs and maintaining monthly progress. 
  • It is the responsibility of the Day Program QIDP to maintain confidentiality with all clients and their families and maintain a professional image/role as a representative of the agency.    
  • Other duties as assigned by the Coordinator of Community Employment Services. 
  •  Ensure compliance with Clearbrook’s Personnel Policies as they relate to the programs.

Qualifications:
Must be legally eligible to work in the United States

Education: 
Minimum: Bachelor’s Degree in Human Services or related field

Experience:
Minimum:      At least 1 year of related experience                           
Preferred:     One year work experience persons with disabilities in a program type setting creating activities and managing caseload for 30+ individuals                                                

Physical Demands:
Must be able to lift 50 lbs.
