- Build strong and trusting relationships with older adults and individuals with disabilities to identify their unique challenges and needs.
- Work closely with communities with lived experience and community partners that serve the community and identify challenges and strategies to address these challenges and needs.
- Support city planning initiatives by ensuring, where possible, that goals and objectives address the needs of older adults and individuals with disabilities.
- Conduct outreach efforts to promote resources and services targeted at older adults and individuals with disabilities. Promote opportunities for community engagement in city initiatives, including participation in planning processes and Boards, and Commissions.
- Collaborate with municipal staff, community partners, and organizations to design and implement programs focused on improving quality of life, including health and wellness initiatives, social activities, mobility support, and accessible transportation options.
- Work with staff to organize workshops and educational sessions tailored to the needs of older adults and people with disabilities, such as health screenings, social-emotional support groups, and wellness workshops.
- Collaborate with the Medford Council on Aging (Senior Center), disability advocacy groups, healthcare providers, and local governments to expand outreach efforts and create a network of support for these communities.
- Collaborate and collect data with the Senior Center to organize a phone tree for senior volunteers checking in on and supporting older seniors and homebound seniors and sharing the impact of social support systems aimed at improving the quality of life for seniors.
- Ensure that all programs are inclusive, culturally responsive, and designed to meet the unique needs of diverse communities, particularly those from marginalized or vulnerable backgrounds.
- Attend and participate in weekly team meetings with other social resilience Connectors, Community Liaisons, the Project Coordinator, and the Manager of Prevention and Outreach.
- High School and GED
- Three (3) years of experience in Project Management
- Associate’s degree or Bachelor’s Degree
- Proficiency with networking, zoom, social media, and other outreach methods.
