The Position: Outreach Nursing Liaison RN
The Outreach Nursing Liaison is a Registered Professional Nurse who will will travel between adult homes to assist our staff and administrators in collaboration with MD’s to assess for new admissions, hospitalizations, residents that need CHHA service.
*Salary commensurate with experience
