Our Nurse Practitioner round primarily in Skilled Nursing Facilities.

Job Description:
Collaborate with patients and various multi-disciplinary teams to develop tailored treatment plans
Utilize evidence-based guidelines to provide optimal care
Conduct patient assessments, diagnoses, and treatments
Create and implement weekly rounding schedules for assigned facilities
Maintain accurate and thorough medical records

What is required:
Current unencumbered Nurse Practitioner license (Illinois)
Nurse Practitioner (required)
