About the Role
  • Provide high-quality physical therapy services to patients in their homes, promoting independence and improving their overall health and well-being.
  • Work collaboratively with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify areas of improvement and develop effective treatment plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
What We Offer
  • A competitive salary range of $95,000 to $115,000 per year, based on experience.
  • A flexible schedule that allows you to self-schedule your visits and work at your own pace.
  • Mileage reimbursement and company car options.

Qualifications
  • Master's degree in physical therapy from a CAPTE-accredited program.
  • Current licensure to practice as a physical therapist in the state of operation.
  • Valid CPR certification.
  • Minimum nine months of experience as a physical therapist, with preferred experience in community or home health settings.
