As a Hospice RN Case Manager, you will play a crucial role in helping patients and families navigate this important time with the support they deserve.Why Choose Elara Caring?Autonomy: Manage your schedule with flexibility and independence.One-on-One Care: Build meaningful connections with each patient you serve.Supportive Environment: Work in a collaborative, encouraging team culture.Competitive Pay: Receive a robust salary package, with bonus opportunities and mileage reimbursement.Professional Growth: Access tuition reimbursement to continue your professional education.Comprehensive Benefits: Choose from medical, dental, and vision plans.Work-Life Balance: Paid holidays, bereavement leave (including for pets), and ample PTO.Pet Insurance: Coverage for your furry family members.Key ResponsibilitiesIn this role as a Hospice RN Case Manager, you will:Assess and address the physical, psychosocial, and environmental needs of hospice patients in their homes.Conduct thorough initial and ongoing evaluations of patients’ nursing needs at each visit.Provide regular health updates to clinical supervisors, physicians, and the care team.Develop and implement personalized care plans, guiding and supporting patients and their families.Maintain quality of care by following individualized plans focused on achieving positive health outcomes.Identify and respond to any changes in a patient’s health, taking timely action as necessary.Ensure care aligns with each patient’s Plan of Care, following physician orders and clinical best practices.Take on additional duties or projects as assigned.What You’ll BringGraduate of an accredited Nursing program (Certificate, Diploma, Associate, or Bachelor’s).Current State RN license.Minimum of 1 year of clinical experience, with hospice or home health experience preferred.Strong nursing skills and familiarity with accepted nursing standards.Ability to manage multiple priorities, deadlines, and documentation requirements.Reliable transportation for home visits.Highly preferred to live in or near the service areaStart Your Journey with Elara Caring TodayWhen you join Elara Caring, you become part of a mission to provide compassionate care in the comfort of patients’ homes.
