Purpose
The purpose of the Initial Competency Evaluation is to confirm that new CRH employees and internal transfers have the minimum competencies required to do the job and to verify compliance with behavioral standards and expectations. This form should be filled out by the leader and signed by both the leader and employee within the employee s first 90 days in the new job.

Minimum Competencies/Essential Duties and Responsibilities
Employee meets the minimum requirements of the following competencies:
oWorks under the direction of Clinical Dietician to Provides individual nutrition consultation to clients.
oOrganizes and plans for individual consultations including preparation of educational materials.
oMaintains required records including nutrition consultation and history forms. Documents all nutrition care provided to inpatients electronically via Epic.
oEstablishes and maintains effective communications with Manager, Chief Clinical Dietitian, physician office staff, and clients.
oCoordinates nutrition information to physician offices and community.
oTeaches Diabetic classes to in-patient and out-patient clients.
oSpeaks to community groups on nutritional topics as requested and approved by the Manager and Chief Clinical Dietitian.
oPerforms all other duties as assigned.
Employee meets all minimum competency requirements of position.

Employee meets behavioral standards and expectations as set by *** s Standards of Service and administrative policies.
Manager Recommendation
If No was selected for any of the responses above, leader must select option 2 or 3 below:
1. Conclude initial competency evaluation period and continue employment.
2. Extend initial competency evaluation period, noting the following in the Manager Comments box below: Re-evaluation date, less than or equal to 90 additional days, which must include an action plan (ICE Action Plans must be completed on a Feedback - Coaching form within Performance Manager, consisting of relevant actions, measures and dates).
3. Dismiss employee for not being able to satisfy the minimum competency requirements or the behavioral standards and expectations of the job (Must discuss with HR prior to dismissal).

SUMMARY
Under the direction of the Chief Clinical Dietitian, this position provides nutrition assessment and care for designated patient populations. This position is responsible for the coordination of nutrition care including nutrition assessment and recommendation, communication with other members of the interdisciplinary care team, education of patients and families when determined appropriate, and documentation in the patient medical record. Nutrition Care is delivered in a variety of settings including inpatient units, inpatient classes, outpatient clinics and classes, scheduled individual outpatient visits, community presentations and programs. The customers of the position include inpatients, outpatients, patients families, physician office staff, physicians, and community members. Ages of customers range from infants to the elderly. The position participates in the Clinical RD Team and the Food and Nutrition Services department. The hours are primarily day shift, with rotating every 5th or 6th weekend coverage and occasional evening hours. This position does not provide direct patient care.

Education and/or Experience
oBachelor's degree (B.A.) from a four-year college or university; with a fifth year internship completed with an accredited institution, or have completed a Coordinated Undergraduate Program in Nutrition and Dietetics.
Certifications, Licenses, Registrations
oRegistration Eligible with the Academy of Nutrition and Dietetics required.
Language Skills
oAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
oAbility to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability
oAbility to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
