Registered Nurse- Case manager

Must have OASIS and previous Home Health experience.The Nurse Care Manager I coordinates the care and service of selected patient populations throughout the acute care episode, in order to facilitate optimal transitions of care. Using a biopsychosocial approach, the role works collaboratively with inter-disciplinary teams, both internal and external to the organization, to enhance the quality of clinical outcomes and patient satisfaction while managing healthcare resources.
Education:Graduate of an accredited nursing school. BSN preferred.
Experience: A minimum of three years registered nursing experience in a clinical setting is preferred. Nursing experience in an acute care setting is beneficial.
Licensure/Credentials: Current Washington State RN license. BLS certification
WA RN license required, Experience with EPIC preferred. Case management and prior acute care experience needed.
Charting System: Epic Systems ,Corporation - EpicCare ,Inpatient - EMR 2014 ,Certified EHR Suite
