Schedule: Full Time, Monday through Friday Responsibilities: Manage and coordinate patient care for hospice patients. Conduct thorough patient assessments and develop personalized care plans. Utilize medical terminology and physiology knowledge to deliver superior patient service. Facilitate smooth transitions for patients through effective discharge planning.
