Shift: 8:00 AM - 4:30 PM

Experience:

• 12 months of Case Management experience is required.

• Experience with Epic is required.

Requirements:

• Candidates must have a New York license in hand at the time of application.

• Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

Pre-employment modules may be required for this role. Please upload any certifications or health documents you have to your profile to expedite your on-boarding process.

Additional Details:

All candidate submissions are at D/E/N ROTATING SHIFT SCHEDULES, Every Other Weekend (EOW), Holidays/On-Call/Callback/Charge per Unit. They cannot approve straight day shifts.

Parking is not provided - this will be an out of pocket expense.

Costs for your 1st Day: Safe Medicate Exam- $10.00— Safe Medicate Exam- $10.00—The fee is $10 and is valid for 30 days. Regardless of practice setting or role, all nurses will complete the safeMedicate assessment during orientation. Password information will be provided at that time. ID Badge- $25.00 - Accepted method of payment: Cash, Check, or Credit Card

Parking Pass- Accepted method of payment: Credit Card ONLY (register online) Day 1- $6.00 (parking garage) By the month- $37.00 Per 13-week contract- $111.00

Mask Fit Test (IF APPLICABLE) - $60.00 - Accepted method of payment: Credit Card ONLY

Safe Medicate Exam, ID Badge, and Mask Fit Test are reimbursable.

Phone prescreen must be completed by clinician in order to receive offer. Please be aware to answer calls from 520 area code.

Vacation WILL NOT be granted during the weeks of Christmas and New Year’s.

With the holidays upcoming, Nursing is offering out 16 week contracts to start Please provide RTO for at least 16 weeks from your start date.

Schedule- Monday – Friday, weekends per need

Certified Home Health Care Experience Required

EPIC or Netsmart Required

One year of experience as a clinical nurse in a hospital required, and experience in home care preferred

2 references (Any reference type)

Proof of driver’s license required to apply
