The Licensed Practical Nurse (LPN) will be responsible for providing high-quality patient care in a dynamic and supportive environment at Avera Oahe Manor. As a key member of the healthcare team, the LPN will work closely with physicians, nurses, and other healthcare professionals to deliver exceptional patient outcomes.
Responsibilities- Assess, plan, and implement individualized patient care plans to meet the unique needs of each patient.
- Administer medications and treatments as prescribed by physicians, and monitor patient responses to ensure optimal outcomes.
- Collaborate with the interdisciplinary care team to develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients.
- Provide education and support to patients and their families to promote health, wellness, and self-care.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Graduate of an accredited school of nursing with a current LPN license in the State of South Dakota.
- Basic Life Support (BLS) certification required.
- 3-5 years of experience in a healthcare setting, preferably in a long-term care or skilled nursing facility.
The LPN will work a variety of shifts, including day and night shifts, with occasional weekend shifts. The schedule will be determined in collaboration with the Director of Nursing and may be subject to change based on the needs of the facility.
PayThe hourly rate for this position is $35.00, with opportunities for advancement and professional growth within the organization.
