LOCATION: Swainsboro, GA (On-site); Region 2

The HUM Navigator provides appropriate administration and coordination for Behavioral Health Individuals throughout the continuum of care. The HUM Navigator will also offer support, education, and other assistance to at-risk individuals to identify and remove barriers to obtaining community-based treatment. HUM Navigation provides consultation and expertise to ensure the course of care is coordinated, scheduled, and communicated through the program process to confirm all program participants are managed appropriately.

The HUM Navigator will have a solid line reporting to a provider within the DBHDD Contracted Network. The HUM Statewide Coordinator, a DBHDD Employee, will have statewide oversight of the program, the ability to provide consultation, training, support and ensure alignment of the work performed by the navigators stays with the original intent of Georgia Department of Behavioral Health & Developmental Disabilities - Division of Behavioral Health.

The HUM Navigator will provide client advocacy, case management, linkage and/or referrals to mental health/substance use services, housing referrals, and all other supportive services as needed. The position is expected to provide and coordinate individualized care and resources coordination.

Position Requirements

  • Employees will attend all mandatory Georgia Department of Behavioral Health & Developmental Disabilities (DBHDD) sponsored trainings, orientations, and workshops which will address various topics and populations including but not limited to Youth, Adults, Crisis, Substance Use, and Recovery
  • Employees will successfully complete a Beck Cognitive Therapy Training within complete in the timeframe specified by the department

THIS POSITION IS LOCATED WITHIN SPECIFIC PROVIDERS WITHIN THE DBHDD CONTRACTED NETWORK THE STATE OF GEORGIA DEPARTMENT OF BEHAVIORAL HEALTH & DEVELOPMENTAL DISABILITIES IS NOT INVOLVED IN REVIEWING OR RECOMMENDING OF ANY APPLICATIONS.


  • Identified providers with BDHDD Contracted Network will be responsible for ALL recruitment and hiring of Navigator Positions.


Qualifications:

  • Maintains a current working knowledge of services available in the community; maintains effective working relationships with community partners, employers, parole/probation officers, social services agencies, schools and other resources in the community to facilitate services for individuals
  • Comfortable, engaging with non-traditional audiences in non-traditional settings; Overall flexibility as to work environment, acuity and needs of individuals / persons served
  • Sensitivity to issues related to homelessness, mental illness, substance abuse, poverty and social marginalization
  • High level of Oral, Written and Technical Communication
  • Ability to perform frequent local, and intermittent statewide travel
  • Exceptional Organizational, Time Management, Creative Problem-Solving Ability
  • Capability to effectively work independently and collaboratively as part of a multi-disciplinary team
  • Knowledge and understanding of (or ability to quickly develop) wide array of local resources available to consumers.
  • Knowledge and understanding of (or ability to quickly develop) DBHDD and Medicaid Policies & Procedures

Educational Requirements:


  • Bachelor's degree in a related field is required, a Master's degree is a plus. (Related fields may include but are not limited to: Psychology, Social Work, Counseling, Public Health, Public Administration, Healthcare, Education, or other Social Service areas of study.)

Required Experience:

  • A minimum of 2 years of documented care coordination of at-risk populations, all disability groups to include but not limited to: Substance Use Disorders, Co-Occurring Disorders, Behavioral Health, and Physical Health.
  • Comfortable with conflict and skilled in conflict management resolution.
  • Comfortable working with individuals of diverse cultures, socio-economic statuses, and/or those in under-resourced communities etc..

Preferred Experience:

  • Significant previous experience working with culturally diverse populations.
  • Strong working knowledge of healthcare systems specifically Behavioral Health and Community Resources.
  • Documented experience in program development and relationship development within a healthcare system.
  • Previous experience in project management or case management.
