Under the direct supervision of the Program Manager, the Clinical Case Manager coordinates and facilitates the Child and
Family Teams (CTFs), leads the development and implementation of individualized family plans, and provides direct services to children and families. The Clinical Case Manager serves as a role model for the wraparound process, demonstrating particular commitment to family safety, unconditional care, family voice and choice, team-driven decision-making, strength-based planning and interventions, flexible services and supports, and community partnership and collaboration.


ESSENTIAL RESPONSIBILITIES AND DUTIES: Other duties may be assigned as needed

● Case Management

  • Weekly home visits
  • Treatment plan monitoring
  • Advocacy
  • Referral and linkage to other needed services
  • Crisis intervention and stabilization
  • Updating progress notes
  • Updating all documentation and forms in a timely manner
  • Coordinates both internal and external services in support of meeting identified family needs

● Collaboration

  • Regular connection with school, doctors, service providers, etc.
  • Creative service delivery connection (community resources)
  • Attends weekly staff meetings, supervision, clinical staff meetings, and clinical consults as scheduled.

● Facilitation

  • Planning, coordinating, and facilitating regular Family Team Meetings (FTM)
  • Supporting engagement of team members
  • Provides leadership in the overall assessment, development, implementation and evaluation of individualized family plans
  • Records and distributes FTM notes
  • Support family in identifying goals and creating family plan
  • Creates individualized safety plans as necessary


Education/Experience:
● Master’s degree (Master’s preferred) in Social Work, Psychology, Counseling, or Marriage and Family therapy
● Two (2) years experience in case management, family services, or a related field of work preferred.
● Strong understanding of trauma informed care and attachment theory

Position/Program Requirements:

● Must possess a valid California driver's license, personal automobile insurance, and clean driving record.
● Must be physically and mentally fit. Must be willing to complete a Tuberculosis (TB) and drug screening test.
● Must be willing to complete a personal background investigation conducted by the State of California.
● Must be flexible to work nights and weekends, may need to work a split-shift, varying schedule according to operational needs.
● Preferred in-depth knowledge of Wraparound principles and practices.
● Performs all duties in a manner consistent with the principles and values of the agency, while adhering to applicable professional codes of ethics, the agency’s policies and procedures, contractor requirements, and regulatory requirements.

● Excellent organizational, analytical, communications skills


Job Types: Part-time, Full-time

Salary: $26.00 - $33.00 per hour
