Summary

The Supervisor of Case Management is responsible for the leadership and functioning of the Case Management Team serving single women experiencing homelessness. Oversees case managers and drives performance to support care delivery to help clients obtain stable income and housing. Adheres to budgets, monitors program goals and evaluations, delegates responsibilities, and assists the manager with maintaining policies and procedures. Represents the program and organization both on-site and throughout the community.

Essential Functions

  • Recruits, recommends candidates for hire and onboards the Case Manager II, and Interns/ volunteers in the team to implement best practices in Social Services and Case Management
  • Facilitates/conducts trainings; assigns tasks and monitors work performance; writes and delivers performance appraisals; recommends salary actions; recommends and administers disciplinary actions; recommends termination of employees and engages and retains a high-performing, accountable workforce
  • Manages a small caseload of clients, providing case management services as needed
  • Monitors and evaluates the work of staff to include, caseload assignments, client interaction, interventions, referrals, and timely documentation of services
  • Plans, coordinates, and facilitates client orientation, meetings, interventions, and events
  • Creates and communicates work schedules and staff meetings to the department and other programs; participates/contributes to MAS meetings (multidisciplinary approach to services) and meetings with the funding source regarding compliance and updates.
  • Prepares, presents, and timely documents client interactions and case plans; participates as a member of the multidisciplinary approach to services team that prepares and provides client case presentations.

· Utilizes a strength-based / trauma-informed approach to services, uses Motivational Interviewing techniques, and CREED to encourage positive change.

  • Maintains CSTAR and CLARITY client databases, provides reports as needed, and updates goals in ADP bi-monthly throughout the year
  • Tracks expenses for the program and communicates discrepancies to the Manager of Case Management.
  • On-time completion of assigned training and policies.
  • Performs other duties as assigned.

Qualifications

  • Bachelor’ degree in social services (social work, psychology, sociology, etc.), or equivalent experience; and four (4) years of experience working with underserved populations in a social service setting; and two (2) years of case management experience.
  • Bilingual (English and Spanish) required.
  • Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations and Prevention and Diversion practices, Housing First, social services, case management, eviction prevention, and crisis intervention.
  • Basic user of MS Office.
  • Ability to pass fingerprinting and background checks upon employment.
  • Must have a valid California Driver’s License and be able to qualify for insurance coverage.
