IHH Care Coordinator

Key Responsibilities include:
- Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism and objectivity.
- Adhere to all confidentiality requirements and regulations as outlined in CARF policies and procedures.
- Establish and maintain routine collaborative meetings with system partners to promote partnerships toward achieving benchmark standards and improving patient care.
- Participate in Quality Improvement activities, including data collection, tracking, and analysis.
- Maintain an active work load in accordance with minimal caseload requirements, clinical quality standards and quality management plan protocols.
- Conduct in-depth interviews/assessments and use specialized knowledge to gather complicated information from patients to be enrolled in the Integrated Health Home.
- Synthesize information from treatment records, risk stratification, and other sources to assess program enrollees. Apply evidence-based clinical guidelines for behavioral and physical health issues, in addition, knowledge of care coordination techniques in assessing the needs of recipients and determining interventions to address those gaps.
- Demonstrate excellent customer service and collaborate with physicians, consumers, members, visitors, staff, and the health care community.
- Collaborate with the Integrated Health Team and treating providers in developing and implementing care coordination plan to address treatment barriers for each low to moderate risk patients.
- Record and maintain documentation. Maintain patient confidence and protect operations by keeping information confidential.
- Demonstrate flexibility in job duties and schedule in order to aid in better serving the organization, the consumers, and to assist reaching benchmark standards. Allow for flexibility in scheduling to accommodate crisis situations for patients. This includes driving to meet patients at their locations in the community within the hours of 7am-7pm, Monday thru Friday.
- Attend regularly scheduled meetings and training sessions to enhance team performance.
- Adhere to all policies and procedures set forth by Prairie Ridge. Complete all necessary paperwork for insurance and billing. Abide by all deadlines set forth by Prairie Ridge.
- Uses available data, such as assessments scores, participant evaluations, and service observations to identify health and wellness needs to develop care coordination plans.
- Participates in various workgroup and committee activities to support departmental and organizational efforts to improve the quality of services throughout our systems of care.
- Regularly utilizes skills such as problem solving, quality management, sound and ethical judgment, planning, organizing and interpersonal skills.
Qualifications and useful skills:
- Bachelor Degree in a human service field. (required)
- Knowledge of, and experience in, working with adults with mental illness. (required)
- Understanding of the mission of Prairie Ridge and the patients we work with.
- Proficiency in oral and written communication skills as well as record keeping.
- Technology skills allowing for proficiency in the use of an electronic health record.
- While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools or controls; and talk or hear. The employee frequently is required to walk and stoop, kneel, crouch, or crawl. The employee is often required to stand, reach with hands and arms, and climb or balance.
- The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 35 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.
- The employee must possess a valid driver’s license, liability insurance policy, and a reliable means of transportation.
