Position Summary:

The Home Health Therapy Manager oversees and manages the delivery of therapy services in the Home Health program, including Occupational, Physical, and Speech and Language Therapy. This role ensures compliance with program rules and physician orders, coordinates a multidisciplinary team, and focuses on continuous improvement of patient care. The Therapy Manager will spend a portion of their time in direct observation of clinical staff, while also engaging in mentorship, training, and problem-solving to maintain high standards of service. Additionally, this role involves creating monthly clinical work schedules, approving PTO requests, ensuring adequate coverage, managing tasks in the electronic medical record (EMR) system, and providing bi-annual performance reviews for therapy staff.

 

Key Responsibilities:

Compliance and Oversight:

  • Ensure the delivery of home health services aligns with program and payer rules as well as physician orders.
  • Facilitates care conferences to verify utilization, compliance with documentation requirements, coverage rules, qualifying criteria, and Medicare Conditions of Participation.

EMR Management:

  • Manage and complete tasks in the electronic medical record (EMR) system in a timely manner.

Team Coordination:

  • Review, plan, coordinate, and manage the activities of a multidisciplinary team to ensure the provision of high-quality patient care services.
  • Provide clear performance expectations and measure adherence to best practices.

Evidence-Based Practice:

  • Utilize evidence-based practice and implement standards that drive practice improvement initiatives.
  • Coordinates field time directly observing clinical staff to ensure adherence to practice standards.

Performance Focus:

  • Maintain a consistent focus on Key Performance Behaviors (KPBs) to drive favorable clinical and functional outcomes.
  • Provide staff with tools, education, and support to achieve best practice standards.

Mentorship and Training:

  • Coach and mentor staff, assist with onboarding and orientation, and address routine service delivery issues, crises, or other critical situations.
  • Assist in the training and development of staff to meet KPBs and implement care delivery best practices.

Performance Reviews:

  • Conduct annual performance reviews for therapy staff, providing constructive feedback and setting goals for professional development.

Quality Assurance: Performance Reviews:

  • Serve on the agency’s QAPI/Compliance Committee and provide follow-up training to staff based on QAPI outcomes.
  • Monitor field-based care delivery for competence and compliance.

Student Engagement:

  • Coordinate internship and shadowing experiences for students to enhance their learning and position the organization as an employer of choice.

On-Call Responsibilities:

  • Participate in the clinical management on-call rotation, providing support and guidance as needed.

Support and Communication:

  • Be available to clinicians for questions and support, ensuring effective communication and problem resolution.

Scheduling and Coverage:

  • Create and manage monthly clinical work schedules, ensuring adequate coverage for patient care services.
  • Approve PTO requests and manage staff leave to maintain optimal staffing levels.

Safety and Environment:

  • Promote safe work practices and environments to protect the safety and well-being of both employees and patients.Additional Duties:
  • Perform other duties as assigned by the supervisor.

 Qualifications:

  • Education: Bachelor’s, Master’s or Doctorate degree in Occupational Therapy, Physical Therapy, Speech and Language Pathology, or a related field.
  • Licensure: Current and valid licensure in the respective therapy field as required by state regulations.
  • Experience: Minimum of 2 years in a therapy role, with at least 1 year in a supervisory or managerial position within a home health or similar setting.
  • Skills: Strong leadership, organizational, analytical, and interpersonal skills. Proficiency in managing and supporting clinical staff and problem-solving.
  • Knowledge: In-depth knowledge of therapy practices, home health regulations, and quality improvement processes.

Additional Information:

  • Work Environment: The role involves both office-based and field-based responsibilities. Travel within the service area may be required in vared weather conditions.
  •  Exposure: May include exposure to blood and bodily fluids.
  •  Physical: Regular use of a keyboard and hands to finger, handle, and feel. Occasional medium lifting may be necessary.
