SUMMARY:

Frontline public health worker who is a trusted member of, and/or has an especially close understanding of the community served. Serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Provide first language access to community resources in Teton County.

a.     Facilities and supports access to social services that aid clients in navigating education, employment, food, and other medical resources.

b.     Provides verbal and written Spanish/English translation of documents and materials for outreach events.

c.     Attends outreach events on nights and weekends to assist in ensuring subsets of our community to promote Health Department programs          and services.

SUMMARY:

PRIMARY DUTIES AND RESPONSIBILITIES:

Essential duties and responsibilities may include, but are not limited to, the following:


  • Promotes and supports access to health programs, such as chronic disease screening and management services, and health insurance enrollment whenever appropriate.
  • Promotes and supports access to social services that aid clients/patients in navigating education, employment and workforce development resources, food insecurity and supplemental nutrition programs and mental health services.
  • Assists in qualitative data collection such as focus groups and key informant interviews.
  • When needed, collects data as required by a grant. This may include completing necessary evaluation forms or using instruments developed by the evaluator and providing necessary information in a timely manner.
  • Meets regularly with the Public Health staff to assist in developing and implementing plans; works closely with the Public Health Specialist to build curriculum.
  • Collaborates with community partners and assists with meetings; strategizes and plans workshops.
  • Supports the relationship between the Teton County staff and the general public by demonstrating courteous and cooperative behavior when interacting with visitors and County staff; maintains confidentiality of work-related issues and County information; performs other duties as required or assigned.


MINIMUM QUALIFICATIONS:

Education and Experience:

  • High School diploma or GED
  • 1-3 years of work in a social service or medical work area of service
  • Or equivalent combination of education, training, and experience


Required Licenses and Certifications:

  • Must possess or ability to obtain a valid State driver’s license and maintain an acceptable driving record
  • Community Health Care Worker Certification within 6 months of hire
  • Community or Medical Interpreter Certification within 6 months of hire


Required Knowledge of:

  • Spanish/English bilingual language skills - additional language skills are desirable
  • Strong communication skills, interpersonal skills, service coordination advocacy skills, organizational skills, health topic knowledge building
  • Standard office practices and procedures, including communication etiquette
  • Basic knowledge with computers, standard MS Office, internet, Google drive
