We're seeking a highly motivated Social Worker to join our Care Management team and play a vital role in supporting patients on their healthcare journey. You'll work closely with patients, families, and the interdisciplinary team to ensure patients receive the social, emotional, and practical support they need to thrive at home or transition smoothly to other care settings.


What You'll Do:

  • Conduct comprehensive assessments to identify patients' social and emotional needs.
  • Develop individualized care plans in collaboration with patients, families, and the healthcare team.
  • Provide counseling and support to patients and families coping with illness, loss, or difficult adjustments.
  • Connect patients with community resources and advocate for their needs.
  • Lead support groups and educational workshops for patients and families.
  • Collaborate with colleagues across departments to ensure seamless care coordination.
  • Continuously monitor and improve your practice through documentation and self-evaluation.


What We're Looking For:

  • Master of Social Work (MSW) degree from an accredited institution.
  • Licensed Social Worker (LMSW/LCSW) in New York State.
  • Minimum of two years of experience in an acute care hospital setting or a related field.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients and families of diverse backgrounds.
  • Excellent assessment and problem-solving skills.
  • Compassionate and empathetic nature with a commitment to patient-centered care.
  • Ability to work independently and as part of a team.
