Assist in activities, that are, social, intellectual, emotional, and spiritual. These programs are in accordance with long-term care residents' assessment and care plan. Mount Carmel Rehabilitation and Nursing Center, in Manchester, NH is a 120-bed facility offering short and long-term care in a warm and supportive atmosphere where residents are cared for according to their individual needs. We are proud to be a part of Catholic Charities New Hampshire’s continuum of care.


This position is Full time - 37.5 hours a week with a every other weekend requirement. A day shift schedule, 8a-4:30p or a 9a to 5pm, depending on the day.

Job Responsibilities;


- Plan, organize and implement daily activities for residents

- Develop and maintain relationships with residents and their families

- Create and distribute monthly activity calendars

- Set up and clean up activity areas

- Ensure all equipment and supplies are available for activities

- Provide one-on-one interaction with residents who require special attention

- Encourage participation in activities and events

- Participate in care plan meetings

- Maintain accurate and complete records of resident participation in activities


Requirements:


  • High school diploma or equivalent.
  • Previous experience working with seniors or in a long-term care setting is preferred.
  • Experience with dementia care.
  • Dementia certification is preferred.
  • Valid driver's license.
  • Ability to drive a small bus, no CDL necessary, will train.
  • Excellent communication and interpersonal skills.
  • Compassionate and patient demeanor.
  • Availability to work flexible hours, including weekends and holidays
