Primary Objective of Position:
The Housing Services Coordinator is responsible for providing services under the Community Supports program for CalAIM. Services include housing navigation, tenancy and sustaining services, day habilitation and other services supporting the needs of those that are unhoused. Supports focused on identifying and maintaining housing.


Major Areas of Accountability:

  • Maintain a caseload of actively enrolled clients within the CalAIM program and as directed by Program Manager.
  • Provide case management services to all clients as needed and in accordance with policies and procedures, including but not limited to intake, assessment, care planning, linkage and consultation to resources in the community.
  • Complete required assessment and individualize housing service plans for all CalAIM participants who are enrolled in the Community Supports program.
  • Evaluates strengths and challenges to addressing short term and long-term goals.
  • Provide supportive services and case planning to address issues and barriers that negatively impact progress toward goals, permanent housing opportunities or retention, and assist in activities that would prepare resident for stable housing.
  • Outreach to tenants through regular home visits and telephone contacts and providing a “listening ear” one-on-one to all tenants.
  • Develop and maintain rapport with clients that will support efforts towards accomplishing their housing goals.
  • Monitor for warning signs of de-compensation and crisis and responding accordingly.
  • Support tenants in connecting to mental health, health, housing, employment, financial literacy opportunities and other resources for homeless individuals.
  • Administrative duties will consist of data entry of all tenant information in HMIS, Cal Optima Connect and agency systems, creating both an electronic and hard file for each tenant in program, and maintain tenant roster of all tenant’s post program.
  • Complete documentation in a timely manner according to program policies.
  • Collaborate with community partners and homeless services providers to coordinate and provide linkage to necessary appointments and services.
  • Attend case management and care coordination meetings with external parties as needed.
  • Other duties as assigned.



Level of Education, Experience and Skill Set:

  • Bachelor’s degree preferred; or equivalent experience serving homeless individuals
  • Over 21 years of age
  • Experience working with individuals experiencing chronic homelessness and mental health challenges
  • Familiarity with the Housing First, Harm Reduction and/or Recovery Oriented service models
  • Knowledge of local Coordinated Entry System (CES)
  • Ability to interact in a culturally diverse community
  • Ability to work effectively in an interdisciplinary team setting
  • Valid California driver’s license and availability of an insured vehicle
  • Job includes some evening and weekend hours and travel throughout California


Physical Demands:
The Housing Services Coordinator is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is required to stand, walk, reach with arms and hands, climb, balance, twist, lean, move from one location to another and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close and far vision. Ability to operate a computer keyboard and lift files and reports. Exposure to glare from a computer.

Work Environment:
Working with the homeless population whether on the streets, in shelters or other places of habitation or services. Exposure to bed bugs or other infestations, unpleasant smells or odors, unclean individuals or homes due to homelessness or mental health symptoms or poverty. Some work will be in a recreational room or common area. The noise level is usually moderate but will vary depending on the activity participating in.
