ESSENTIAL DUTIES:
\tModels a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
\tEnsure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
\tKnowledge of Medicare pre-admissions for qualifying diagnosis and coverage eligibility
\tObtains/ maintains physician certificates for each Medicare resident
\tMaintain good attendance record
\tMaintains MDS schedule
\tCompletes MDS 3.0 for Medicare residents
\tAssures that all MDS information is completed and transmitted per policy, and is compliant with all regulations and forwarded to billing office\t
\tTracks Medicare resident benefits days, validates daily Medicare census and coordinates this information with accounting representative and therapy representative, to ensure accurate billing (if applicable)
\tAssures all quarterly resident assessments are completed
\tAssures all care plans completed and updated as needed
\tCoordinates daily PPS meetings and weekly Medicare meetings
\tReviews nursing documentation of residents and provides guidance/training as needed, to ensure charting is appropriate and meets coverage guidelines
\tResponsible for completing case management to ensure adequate documentation for reimbursement
\tCoordinates discharge of residents
\tWorks with Clinical Liaison to verify appropriateness of admission and admission diagnosis codes
\tMaintains weekly communication with insurance case manager to receive approval for resident treatment and communicate with physician on insurance resident admission for timeliness of physician visits
\tCommunicates with rehab department on routine basis, to discuss therapy services
\tCommunicates with Life Enrichment and Social Services to coordinate assessment in a timely manner
\tRecruit, hire, train, coach, and evaluate departmental staff (management staff only)\tMonitor residents for changes in condition, ensure MD, and family, are notified and appropriate documentation is reflected in the medical record
\tAssess residents initially and annually and complete resident service plans on an annual or as needed, basis
\tMaintains the Residents Roster and Acuity Levels within the department (i.e., State Boards, QA monitor)
\tReview and follow-up all consultant reports in a timely manner
\tReviews monthly physicians orders and ensures accuracy
\tMonitors psychotropic behavior sheets and makes recommendations as needed
\tComplete performance evaluations of all licensed nurses, mediation aides and CNAs
\tNotify Supervisor/Administrator of incident reports concerning abuse
\tThoroughly investigate any complaints and concerns and report to Administrator
\tAttend all assigned committee meetings
\tReview incident report documentation and ensures follow-up according to facility policy
\tMake rounds to verify clinical compliance and quality of care and provide coaching and disciplinary action as indicated within the facilities policies and procedures
\tEnsures all Life Enrichment programs/QA monitors are implemented on assigned units
\tPerform change nurse duties, in cases of staffing emergencies, as required to meet the residents immediate needs
Other duties as assigned by Director of Nursing
MINIMUM QUALIFICATIONS:
\tValid Texas RN or LVN license
\tAANAC RAC-CT Certification a plus
\tIn depth knowledge and demonstration of MDS skills
\tMinimum of 2 years experience with RAI process/MDS/PPS/Medicaid CMI in a Medicare/Long Term Care setting Minimum of 1-2 years' experience as ADON Experience with electronic medical charting\tKnowledge of SNF, Medicare documentation and reimbursement guidelines
\tDemonstrated computer competencies
\tKnowledge of rehabilitation theory and application
\tKnowledge of current professional nursing theory and practice
\tAbility to read, write and effectively communicate in English
\tExcellent verbal and written communication skills
\tKnowledge of restorative nursing care practices
\tDemonstrated supervisory and managerial skills
\tAbility to maintain a stable disposition in performing work in association with staff and residents
\tAbility to maintain a high level of performance in an environment of frequent demands and changes
\tAbility to perform Essential Duties
\tGenuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients
\tAbility to perform work tasks within the Physical Demand Requirements
\tAbility to comply with the Resident Bill of Rights (LTC) and Employee Responsibilities
\tAbility to comply with organizational and departmental safety policies and procedures
