**Responsibilities:**
- Provide direct patient care in accordance with the highest professional standards and practices.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and assess residents' health and update patient records accordingly.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans.
- Ensure a safe and clean environment for residents and adhere to infection control protocols.
- Communicate effectively with patients, families, and other healthcare team members.

**Qualifications:**
- Current LPN license in the state of Indiana.
- CPR certification.
- Proven experience in a skilled nursing or long-term care setting is preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively in a fast-paced environment and adapt to changing priorities.
